SoAHEC completed its fourth year and is now working on the fifth year of a multi-year community-based intervention for Doņa Ana and Otero Counties. Year One focused on training bilingual Spanish speaking trainers in Que Sabrosa Vida and the Stanford School of Medicine Chronic Disease Management Model. Year Two implemented a Peer Education Model which trained 11th and 12th graders to teach diabetes prevention education to 4th and 5th grade elementary children. Year Three focused on assisting community coalitions in addressing the need for more physical activities and healthier food choices at cafeteria and vending foods at schools. During year three, DPI also worked to collaborate with grocery stores and restaurants in helping provide consumer information identifying foods that are high in fiber, vitamins and minerals and low in sugar and fat content. Year Four focused on bringing new life to the Stanford Chronic Disease Self Management Model, community partnerships and with local businesses to share healthful information with patrons. Presently, during Year Five of funding, DPI staff continues to work diligently on completing the evaluation process of the Stanford Chronic Disease Self-Management Model, the evaluation of the Kidz Keep N Fit program, and the provision of the Healthy Lifestyles trainings.Click here to view downloads Diabetes Prevention Initiative DocumentaryProduced by: Kiel Poore and Chris MarinNarrated by: Kiel Poore
