Diabetes Prevention

 


Background                                                                                                                     The Diabetes Prevention Initiative now in its fourth year, is funded by the Paso del Norte Health Foundation.  The initiative focuses all its services in Doņa Ana and Otero Counties.  Year one’s emphasis was on training bilingual (Spanish/English) trainers in Que Sabrosa Vida and the Stanford School of Medicine Chronic Disease Self-Management (CDSM) Program. Year two’s emphasis was a Peer Education Model called From Classroom to Community which trained 11th and 12th graders to educate 4th and 5th grade children about diabetes prevention. Year three’s emphasis included encouraging healthier food options at stores and restaurants, building more community collaboration around school nutrition policies, physical activity, and healthy lifestyles in the community and at home. Year four will include continued CDSMP trainings, a one-year follow-up evaluation of previous SCDSMP trainings, increased collaboration in each county, health literacy and cultural competency trainings, and continued efforts with the diabetes-friendly food program.

Diabetes-Friendly Foods                                                                                                  The Diabetes-Friendly Food Program brings healthy lifestyle messages and useful information to consumers in restaurants and grocery stores willing to participate in Doņa Ana and Otero counties.

Stanford Chronic Disease Self-Management Program                                                   The Chronic Disease Self-Management (CDSM) Program was developed by the Family and Community Medicine team in the Department of Medicine at Stanford University by a team of physicians, health professionals and patients to train, people who suffer from one or more chronic diseases, ways in which to cope and better manage their self care.  During the five-year research project of the CDSM, a randomized study of more than 1,000 participants found that those who completed the program compared to those who did not, improved their healthful behaviors, health status and decreased their days in the hospital. The program is designed for people with chronic diseases such as heart disease, asthma, arthritis, diabetes, bronchitis and others.

Community Workshop:                                                                                                    A CDSM community training workshop consists of 15-20 participants where participants meet once a week for six weeks with each weekly 2 and a half hour session consisting of 5-7 different activities. The trainings in Dona Ana and Otero Counties are provided through the Southern Area Health Education Center (SoAHEC) and are conducted by two qualified trainers. Each training session will provide highly participatory activities, where mutual support and success builds the participants’ confidence in their ability to manage their health and maintain active and fulfilling lives. Community workshops are offered in English and Spanish in Doņa Ana and Otero Counties. Check our calendar of events for upcoming training dates.

 

Lead Training:                                                                                                                 To become qualified to conduct a community workshop trainer, you must attend a SoAHEC/DPI sponsored lead training event.  Lead trainings are conducted by two qualified master trainers who will training and prepare all lead trainers to conduct a thorough and effective community workshop using the Living a Healthy Life with Chronic Conditions manual and relaxation tapes. 

Lead trainings are offered in English and Spanish in Doņa Ana and Otero Counties.  Check our calendar of events for upcoming training dates.

Master Training:                                                                                                             To become a qualified master trainer, you must attend a training conducted by the Family and Community Medicine in the Department of Medicine at Stanford University.
